SAFE CLINIC

It is with regret that we must announce the closure of our very successful and well attended SAFE Clinic.

This is due to the withdrawal of essential funding by the Board of Dacorum Practice Based Commissioning Group despite evidence to show that the clinic provided a discreet, efficient local service for the residents of the whole of Dacorum.

Extended hours appointments

Please visit the appointments page to learn more about extended hours appointments coming into effect from Monday 26th May 2008.

Dr Hillier has now successfully completed his training and is now a fully qualified General Practitioner and we are please that he has remains with us as a full time member of our clinical team.

We are pleased to welcome Dr Kiran Mahmood in the post of GP Trainee. Dr Mahmood is completing her training on a part-time basis and will be with us for almost 2 years.

* GP Registrars are fully trained Doctors who are undergoing further education to qualify as a general practitioner.

Registration

If you wish to register with this practice, please go to the most convenient surgery (Kings Langley or Bovingdon) and request a registration pack. You will be asked to complete a registration form and new patient questionnaire. We ask that all new patients attend a new patient appointment with Nurse in order that we can perform simple health checks and obtain some current information about your health status, prior to receiving your medical records from your previous GP.

The Practice welcomes patients for registration regardless of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation.

The Practice requires a registration form (GMS1) to be completed and signed for each new patient, to provide details of name, address, date of birth, previous address and doctor, and National Health Number (if known). You may also register as a temporary resident (GMS3) whilst working or visiting in the area provided this involves an overnight stay. Please indicate the length of your intended stay as either less than or greater than 15 days. These forms can be obtained from the surgery or by downloading and printing the form. The GMS1 form must be signed by the patient or patients representative and returned to the surgery.

To download the GMS1 form, click here. Please also download an additional details page here. To download the GMS3 form, click here. To read more about patient health checks, click here.
