Practice Area
The practice covers Hereford City and its environs, extending outwards roughly two villages in each direction.
Directions
- Parking is available at the surgery and there are wheelchair ramps for the disabled at both the front and lower doors.
- Buses stop at Tesco's (Victoria Street).
- Pushchairs and bicycles can be left under cover in the front porch.
Registration
If you would like to register as a new patient, please complete the required forms available at the surgery.
New Patient Health Checks
All new patients registering with the Surgery are required to have a health check with the nurse. You will be invited to make the appointment when you register. This health check completes the registration process and will enable the Doctors to have an insight into your medical background whilst awaiting receipt of your full notes, which are forwarded to us automatically by the Primary Care Trust.
Violence And Abuse
The Practice has a no tolerance policy towards violence or abuse towards Practice and Clinical staff. Therefore where circumstances exist that it may be considered reasonable, or in the best interests of the patient, a patient may be removed from the Practice List. The full protocol for removing patients from the Practice List is available upon request.
