Repeat prescriptions can be requested in several ways – By hand to the surgery, fax - 01923 267233, post, or via the form below. Requests can not be taken over the phone. Please allow 48 hours before collecting your request.

Your prescription can be sent to a local pharmacy from where you will be able to collect your medication.

Private prescriptions will need to be paid for on collection, cheques are to be made out to Haverfield Surgery.
