registrationwith the practice
The Practice welcomes patients for registration regardless of disability or medical condition, age, appearance, race, gender, social class, religion or sexual orientation.
The Practice requires a registration form (GMS1) to be completed and signed for each new patient, to provide details of name, address, date of birth, previous address and doctor, and National Health Number (available from previous doctor). In addition we are now required to gather ethnicity data on all new patients and therefore we would ask you to download and complete the patient questionnaire - please hand this in with your registration form. You may also register as a temporary resident whilst working or visiting in the area provided this involves an overnight stay. Please indicate the length of your intended stay as either less than or greater than 15 days. These forms can be obtained from the surgery or by downloading and printing the form. The GMS1 form must be signed by the patient or patient's representative and returned to the surgery.
Repeat prescriptions
If you require repeat medication please book to see the Doctor soon after registering so that you do not run out of your medication. A script can not be given unless you have first been seen by the Doctor.
