We are a dispensing practice and are able to supply medication for all patients living more than one mile from the nearest chemist.

Repeat prescriptions can be requested using the form below. Please complete the form with all the details you can provide and then click "Send" once to send your request to the surgery. A confirmation page will appear if the form has been sent successfully. If there is a problem, the relevant parts to change will be highlighted.

Please note that this form is only to be used to request medications that appear on your medication list that is printed out at the surgery and given to you when you pick up your repeats. If you require other tablets or have other queries then please telephone the surgery.

Messages using this system are collected at the surgery between 8:00-8:30 am each weekday morning. Please allow 48 hours from the time we collect the message for us to prepare the medications for collection.
